They are monitoring what is happening with the nerves during surgery. There is no reason to monitor the nerves before the surgery... it's not like an EKG where you're monitoring the activity of the heart at all times. Several policies (Oxford and others) state "actual time of monitoring begins after the surgery has begun. If the monitoring spans two dates of service, the date reported must be the date the monitoring began."
One unit is 31 to 90 minutes. Two units is 91 to 120 minutes.
